Prior research has indicated that, typically, health-related quality of life recovers to its pre-illness baseline within the months subsequent to significant surgical procedures. Although the average effect on a studied group is considered, it may fail to capture the diverse experiences of individual changes in health-related quality of life. Currently, there is limited knowledge about the variability in health-related quality of life (HRQoL) among patients experiencing stable, improved, or worsened outcomes after major surgical oncology procedures. The research project is focused on describing the manner in which HRQoL shifts over the six-month period after surgery, as well as quantifying the level of regret expressed by patients and their family members related to the decision to have surgery.
Within the University Hospitals of Geneva, Switzerland, a prospective observational cohort study is being carried out. We have selected patients 18 years or older who have undergone gastrectomy, esophagectomy, pancreatic resection, or hepatectomy for this study. A validated minimal clinically important difference of 10 points in health-related quality of life (HRQoL) is used to determine the primary outcome: the percentage of patients in each treatment group who show improvement, stability, or decline in HRQoL six months post-operative. A secondary point of evaluation, performed at six months post-surgery, focuses on whether patients and their family members may have any regrets about their decision to have the surgery. Utilizing the EORTC QLQ-C30, HRQoL is measured before surgical intervention and again six months afterward. We utilize the Decision Regret Scale (DRS) to evaluate regret, specifically six months after the surgical operation. Concerning perioperative data, important factors include preoperative and postoperative residence, levels of preoperative anxiety and depression (as evaluated by the HADS scale), preoperative disability assessed via the WHODAS V.20, preoperative frailty (determined by the Clinical Frailty Scale), preoperative cognitive abilities (measured by the Mini-Mental State Examination), and any pre-existing medical conditions. The 12-month follow-up is part of the plan.
The study received the initial approval of the Geneva Ethical Committee for Research (ID 2020-00536) on April 28, 2020. Presentations at national and international scientific events will detail the results of this study, followed by submissions for publication in an open-access, peer-reviewed journal.

Sub-Saharan Africa observes a marked increase in the discipline of emergency medicine (EM). To determine the current effectiveness of hospitals in providing emergency services, a crucial analysis of their capacity is necessary to uncover gaps and chart future growth directions. Investigating emergency unit (EU) proficiency in emergency care provision within the Kilimanjaro region of northern Tanzania was the aim of this study.
Eleven hospitals providing emergency care in three districts within the Kilimanjaro region of Northern Tanzania were studied through a cross-sectional design in May 2021. To ensure a complete sample, every hospital within the three-district area was surveyed using an exhaustive sampling approach. Emergency physicians employing the WHO-developed Hospital Emergency Assessment tool surveyed hospital representatives. The data was then analyzed, using Excel and STATA.
Emergency services were available at all hospitals during every 24-hour period. In nine facilities, designated areas supported emergency care; four had designated providers connected with the EU. Two, however, failed to implement a system for systematic triage. Concerning airway and breathing interventions, adequate oxygen administration was observed in 10 hospitals, despite manual airway maneuvers proving sufficient in only six, and needle decompression in only two. In all facilities, fluid administration for circulation interventions was sufficient, but intraosseous access and external defibrillation were only available at two locations each. Of all EU facilities, only one had a readily available ECG, and none were equipped to perform thrombolytic therapy. Fracture immobilization was a standard practice in all trauma intervention facilities; however, additional, vital procedures, such as cervical spine immobilization and pelvic binding, were not implemented. A lack of training and resources was the principal cause of these deficiencies.
While emergency patient triage is systematically undertaken in most facilities, notable shortcomings in diagnosing and treating acute coronary syndrome and the initial stabilization of trauma patients were evident. The scarcity of resources was primarily caused by a lack of suitable equipment and insufficient training. The development of future interventions, across all facility levels, is vital for improving training standards.
While most facilities practice a systematic approach to emergency patient triage, areas of deficiency were prevalent in the diagnosis and treatment of acute coronary syndrome and the initial stabilization of patients with trauma. Resource limitations stemmed fundamentally from inadequate equipment and training. Future interventions are vital for upgrading training standards at every level of facility.
Organizational decisions concerning workplace accommodations for pregnant physicians necessitate supporting evidence. Characterizing the positive aspects and shortcomings of current research examining the association of physician work hazards with pregnancy, labor, and newborn outcomes was our primary objective.

Geriatric guidelines highlight the avoidance of benzodiazepines and non-benzodiazepine sedative-hypnotics as a key element of treatment for older individuals. A period of hospitalization offers a significant opportunity for the start of reducing prescriptions of these medications, particularly given the discovery of new reasons for their avoidance. Using implementation science models and qualitative interviews to provide an in-depth portrayal of the barriers and facilitators to benzodiazepine and non-benzodiazepine sedative hypnotic deprescribing in hospitals, we developed potential interventions to address the challenges identified.
The interviews with hospital staff were coded using the Capability, Opportunity, and Behaviour Model (COM-B) and the Theoretical Domains Framework, then, we utilized the Behaviour Change Wheel (BCW) to co-create potential interventions with stakeholders from each clinical group.

During our study, we interviewed 14 medical professionals. We found constraints and supports spread throughout the comprehensive COM-B model domains.
